- $75 Stat Draws (same-day)
- $120 Therapeutic Phlebotomy (with order)
- $40 Specimen Pickup/Delivery Only
Additional Travel (beyond 10 miles):
- + $10 (total $70): Cave Creek, Carefree, Norterra, Deer Valley, Black Canyon City, North Glendale & North Peoria
- + $20 (total $80): Desert Ridge & Paradise Valley; West Valley: Glendale, Sun City
- + $30 (total $90): West Valley: Surprise, Buckeye, Goodyear, Avondale
- + $40 (total $100): East Valley: Scottsdale, Rio Verde, Fountain Hills, Tempe
Therapeutic Phlebotomy is $120 base plus applicable travel fee by location.
